Non-Hospital Crisis and Emergency Support

New Horizon’s Crisis Center (CC), located in Valley Stream, provides a welcoming, private, safe, and professional environment where children, adolescents, adults, and families may walk in to receive immediate, non-hospital crisis and emergency support 24 hours a day, seven days a week, 365 days a year for behavioral health and/or substance use crises.
 
The CC also accepts referrals from first responders and community providers for individuals in need of urgent crisis support. Services emphasize seamless connections to a wide range of community-based resources to help individuals and families stabilize and move toward recovery.
 
In addition to crisis services, New Horizon conveniently offers multiple community-based programs co-located within the same building, including behavioral health, substance use treatment, care management, senior services, developmental disability supports, and peer services—allowing for timely access to ongoing care when needed.
At the Crisis Center, we will:
  • Listen with care and respect
  • Assess your immediate needs and situation
  • Promptly address and de-escalate behavioral health and co-occurring disorder crises, with particular attention to individuals experiencing Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), and co-occurring conditions
  • Connect you to the services and supports you need*
  • Provide immediate, community-based crisis services as an alternative to hospital emergency rooms and inpatient settings
  • Offer referrals and warm hand-offs to ongoing services to reduce risk and support long-term stability and recovery
What Is the Crisis Center (CC)?
The Crisis Center (CC) provides a comprehensive range of services designed to offer immediate support to individuals experiencing behavioral health and/or substance use crises. Crisis care may also address physical health concerns related to substance use, withdrawal, or sub-acute chronic conditions.
 
The primary goal of crisis care is to support individuals in the least restrictive setting possible, helping to prevent unnecessary hospitalization, incarceration, or emergency room use.
 
The CC integrates additional services that promote a whole-person approach to care, emphasizing recovery, wellness, trauma-informed practices, and the integration of physical and behavioral health services.
Who Does the Crisis Center (CC) Serve?
 
New Horizon’s Crisis Center (NHCC-CC) is a 24/7, voluntary program serving adults, children, adolescents, and families who are experiencing a crisis related to mental health concerns, substance use, or life stressors.
 
Individuals may seek support for concerns including, but not limited to:
  • Emotional distress

  • Anxiety

  • Depression

  • Family or interpersonal conflicts

  • Domestic disputes

  • Substance use–related concerns

The Crisis Center provides immediate, compassionate support in a safe, community-based setting.
Walk-Ins and Referrals
The Crisis Center (CC) accepts walk-ins and referrals from a wide range of community partners, including:
  • Mobile Crisis Providers

  • Law Enforcement

  • Emergency Medical Services (EMS)

  • Hospitals and Emergency Departments

  • Primary Care Providers (PCPs)

  • Schools

  • Social Service Providers

  • Local Government Agencies

  • Rehabilitation and Treatment Centers

The Crisis Center is grant-supported by the Substance Abuse and Mental Health Services Administration (SAMHSA), reflecting a commitment to accessible, community-based crisis care.
