For more than 40 years, New Horizon Counseling Center has provided behavioral health services for adults living in New York State Department of Health (DOH) licensed Adult Homes. We provide integrated behavioral health care, including mental health and substance use services, with care and support based on each person’s needs, strengths, choices, and goals.
Our teams work closely with each person and coordinate care with Adult Home staff, primary care providers, and on-site or visiting specialists, community-based services, and other providers so care and supports work together. We provide medication services, treatment for substance use concerns, and 24/7 crisis support.
We help our adult home clients maintain stability, build independence, strengthen relationships and social connections, and work toward goals that matter to them. This may include making the best use of Adult Home and community supports, planning meaningful and enjoyable activities, reconnecting with important people, and exploring more independent living options when desired.
Using evidence-based practices, we help each person build skills, make informed choices, and improve overall well-being and quality of life.
