Assertive Community Treatment (ACT)
Not everyone benefits from receiving services in a traditional office or clinic setting. New Horizon Counseling Center (NHCC) offers Assertive Community Treatment (ACT) to support individuals in environments where they feel most comfortable—such as their home or other community-based locations.
ACT is an intensive, community-based service designed for individuals living with serious mental illness who may face barriers accessing traditional treatment. Multidisciplinary ACT teams provide comprehensive psychiatric, vocational, integrated, and rehabilitative services by meeting individuals where they are—both physically and in their recovery journey.
Each ACT team includes professionals with lived experience who bring valuable insight, empathy, and understanding to the care process. Using a “whole team” approach, ACT delivers coordinated, person-centered, and recovery-oriented care aimed at reducing hospitalizations and promoting stability, independence, and community integration.
ACT services are available 24 hours a day, 7 days a week, and are designed to help individuals build skills, strengthen supports, and achieve meaningful recovery within their communities.
