WHAT IS A CCBHC?

A Certified Community Behavioral Health Clinic (CCBHC) provides comprehensive behavioral health care, including mental health and substance use services. CCBHCs serve people and families of all ages and backgrounds across the lifespan, with care and support based on each person’s needs, strengths, and goals.

The CCBHC model brings services and specialists together to make care easier to access and coordinate. Our teams help with mental health and substance use concerns, crisis needs, medications, recovery, and important health needs. We also connect people with support for housing, benefits, education, employment, transportation, and other areas of daily life.

By bringing care and support together, we help people work toward better overall well-being, greater independence, improved quality of life, and meaningful goals at home, at work, at school, and in their communities.

New Horizon’s CCBHCs follow federal guidelines from the Substance Abuse and Mental Health Services Administration (SAMHSA) and New York State requirements involving the Office of Mental Health (OMH) and the Office of Addiction Services and Supports (OASAS). This helps services work together to support the whole person.
