Outreach team members build awareness of New Horizon Counseling Center’s mission and programming by serving as ambassadors for the organization.
The goal is to form collaborative alliances with other organizations and foster community engagement.
The team seeks to bring disconnected and underserved individuals into care by working with referral sources such as hospitals, primary care physicians, shelters, substance abuse treatment programs, mental health providers, SROs, and social service organizations to find and engage clients.
In every instance, the outreach team works to achieve a mutually beneficial relationship with the referral source through establishing a common goal for New Horizon Counseling Center, the other organization, and for the clients. Some of these partnerships result in team members maintaining a permanent presence at affiliate locations while others ensure that clients stay connected to primary and behavioral healthcare.
With a strong emphasis on community outreach and community integration/participation, New Horizon Counseling Center promotes access to care in the neighborhoods that surround our clinic locations. We also provide referrals to other New Horizon Counseling Center programs such as Health Home and Day Treatment Program.
